Competitor Declaration forms

Prior to competing in any year’s Sprint Championship, each entrant must complete a Competitor’s Declaration Form, giving details of their vehicle (including modifications) and nominating the Class in which they wish to compete.

This form must be signed and lodged with the Club Captain, who will check the details and confirm the competition Class.

Please download the form you need from the options below, complete, scan and e-mail it to competitorforms@mx5vic.org.au. Please note: Competitor Declaration Forms will only be accepted via this e-mail address.

• Standard Classes: Standard NA, Standard NB, Standard NC, Standard ND
• Clubman Class
• Modified Classes: Modified and Super Modified
• Open Classes: Restricted Open and Open

Complaint/Sportsmanship Review form

In the event that you want to lodge a complaint or protest against another competitor’s car or behaviour, you can submit a Complaint/Sportsmanship Review Form. Once completed, please scan and e-mail the form to the Compliance Review panel.

This form must be lodged within 24 hours of the alleged incident, and will be reviewed by the Club’s three-person Compliance Review Panel (chaired by the Club Secretary).
